Our phase-matching system assists your licensed provider in determining the closest clinically appropriate starting phase for your new formulation. Because different delivery methods can be absorbed differently by the body, the exact dose may change while aiming for a similar therapeutic effect.

Questions people ask before switching.

It depends on your vial's concentration, which is why there is no single universal answer. On a standard U-100 insulin syringe, 100 units = 1 mL. If your vial is mixed to 10 mg/mL, a 2.5 mg dose is 0.25 mL, or 25 units. At 5 mg/mL, the same 2.5 mg dose is 50 units. Always confirm your concentration on the vial or pharmacy label, and use our free Dosing Calculator to convert your exact dose from mg to syringe units in seconds.
Tirzepatide is typically started at 2.5 mg injected once weekly for the first 4 weeks. After that, the dose is usually increased in 2.5 mg steps (5 mg, 7.5 mg, 10 mg, 12.5 mg) every 4 weeks or more, as tolerated, up to a maximum of 15 mg once weekly. Your licensed provider determines the right phase for you, and if you're already on tirzepatide elsewhere, we can review your treatment and continue you at your current phase when clinically appropriate, so you don't start over at 2.5 mg.
Divide your dose in mg by your vial's concentration in mg/mL to get the volume in mL, then multiply by 100 to get U-100 syringe units. Example: 5 mg ÷ 10 mg/mL = 0.5 mL = 50 units. If you'd rather not do the math, our Dosing Calculator does the conversion for you.
Semaglutide for weight loss typically starts at 0.25 mg once weekly, then steps up roughly every 4 weeks through 0.5 mg, 1 mg, and 1.7 mg to a maintenance dose of 2.4 mg once weekly, as tolerated. If you're already partway up the ladder with another provider, our licensed providers can review your records and continue you at an equivalent phase rather than restarting you at 0.25 mg.

This is the core of our Phase Matching review: you keep the progress you've earned instead of re-titrating from the starting dose.

If it's been several weeks or months, your provider may recommend resuming at a lower phase so your body can safely readjust before stepping back up.
